Why outcomes matter to us
At Hesley Group, our focus is quality of life, wellbeing and meaningful progress for the people we support. Success is not always about speed or simple milestones, but about stability, understanding, dignity and sustained positive change over time.
How we measure outcomes
We use a combination of clinical formulation, Positive Behaviour Support and recognised quality-of-life measures to understand whether support is making a meaningful difference. Outcomes are reviewed regularly at individual, service and organisational level.


The San MartÃn Scale – understanding quality of life
Aligned to the H.E.S.L.E.Y. Integrated Model, Hesley Group is embedding the use of recognised quality-of-life measures, including the San MartÃn Scale , a recognised quality-of-life assessment tool for people with learning disabilities and intensive specialist support needs
The San MartÃn Scale helps us to understand quality of life across key domains, including:
• Emotional wellbeing
• Physical wellbeing
• Personal development
• Self-determination and choice
• Social inclusion
• Interpersonal relationships
• Rights and dignity
By using this framework, we are able to look beyond immediate behaviour or risk and focus on what truly matters to the person.
How outcomes inform support
Information gathered through the San MartÃn Scale is not used in isolation. It directly informs:
- Clinical formulation and review
- Positive Behaviour Support planning
- Environmental and sensory adaptations
- Education, care and support priorities
- Staff coaching and reflective practice
