Referral, Transition & Moving In

next steps imageBelow, in simple terms, is our process:

Enquiry
We have a dedicated referral department to help with any enquiries. It’s often parents, friends and carers who are the first to get in touch and we aim to ensure they have all the information and reassurance they need. However, to formalise a referral, it will need to be made by the relevant placing authority. This may be a local authority via a placement, social work or SEN (Special Educational Needs) team. The relevant placing authority may also be a primary care trust, a clinical commissioning group, a community health council or someone managing an individualised budget.

Referral
Once we’ve received relevant documents from a placing authority detailing the needs and support requirements of the person being referred, relevant managers and clinicians will review them. If we then feel there is potential to meet the needs of that individual, we will offer an assessment to further explore the suitability of our services.

Assessment
We carry out a free-of-charge assessment to make sure that we can meet a person’s needs. Our specialist staff arrange to meet the person and as many people centrally involved in their support as possible. The assessment considers the whole person and looks at a range of health, social, educational and any other needs. If we believe we can meet these needs, we will offer a place, in writing, to the appropriate placing authority.

Contract
When the placing authority confirms acceptance of our placement offer and funding approval, we send out a formal contract together with necessary documentation regarding the transition and intended support for the person concerned. With funding and contracts in place, detailed transitioning planning can now commence.

Transition & moving in
Any change for someone with autism and/or other complex needs can be very difficult. Moving home can of course be stressful for any of us. The multiple changes that can be associated with such a move, whether in terms of relationships or physical environment for example, can be highly challenging and distressing for someone with complex needs. This can be the case, particularly if moves are poorly planned and supported and if the person’s needs and wishes are not properly considered or kept central to the move. 

The transition process will be managed by a senior manager from the relevant service who will work with the person, their family/carers and the placing authority to ensure the move is properly planned and managed and that the best interests of the person are properly met. If somebody needs to come into our services urgently, we will of course support this by employing the same principles, seeking to ensure the smoothest transition possible. 

Once the person has moved to one of our services we aim to ensure that they are as happy as possible from their very first day with all relevant documents including support plans, risk assessments and service user guides firmly in place.
