Reviews and AdvocacyReviews
Each person has a formal annual review, usually with their family and a funding authority representative. We support people to express their views as a key part of this and, where at all possible, to take part in their own review. An advocate may be involved if required.
A rolling programme of multi-disciplinary case reviews also makes sure people’s care and support needs are continually being met. It also ensures we fulfill our commitment to each person: to always provide the best possible care and support, and continue to evolve this in line with the pathway plan.
Everyone has access to advocacy. We work closely with advocacy providers to support and promote the voice of the individual and advocate for his or her rights.
